What do I need to submit?
Please submit a picture of the tag, picture of the issue, description of the issue, and a complete address to send parts to. **If applicable, please determine what side the issue is on when you are facing the chair (left side facing vs. right side facing).
Why do I need a picture of the tag?
The product tag includes the order ID number which can be matched in our system. Providing this allows us to pull up the necessary details associated with the parts needed.

Where can I find the product tag?
For recliners, look under the footrest. For other products, look under the base.
What example pictures should I include?
Example pictures can include but aren’t limited to: mechanism, handle, arm caps, Q4 pedal, trays, casters, Heat/Massage control box (i.e., lights).
