Warranty Request

Complete the below form to submit a warranty claim. Click “Add” under the Product Issue Information section to create a line for each product. Using the file upload section at the bottom of the form, please include a photo of the ID Tag and of the product issue, selecting the corresponding option from the File Type dropdown menu (after files are uploaded). If there are multiple products on your claim, please name product photos based on the ID tag number. Warranty claims may not be able to be processed without an ID Tag.

**NOTE: The ability to upload videos (for product motion issues) will be available mid-June**
